l Native Japanese Instructors
l Saturday Class
l Maximum 25 students per class (no minimum)
l Eligibility: Anyone 16 years old or older
CLASS LOCATION: Ted Polk Middle School, 2001 Kelly Blvd, Carrollton, TX 75006
(on Kelly Blvd. between Keller Springs and Belt Line)

HOW TO SIGN UP:
Please download registration form below and send your registration form and tuition to the address below by July 28th, 2017. You may also submit your registration form and check (or cash) directly at our office. (by appointment only) You must register and pay tuition before attending a class.

TUITION: Full payment $320 / Installment payment: $170 x 2 = $340
l The tuition must be paid in full by July 28th, 2017.
** If you wish to make payment by installment, the tuition will be $170 X 2= $340.
l $30.00 for one-time Registration Fee (for new student only).
l Checks payable to Dallas Japanese Association.
l Tuition is not refundable after first 3 classes. Registration fee and textbooks are not refundable.
